Admissions/Eligibility
Criteria
The
Freeman Center provides services for both male and female adults, who meet the
following criteria and:
1.
are eighteen years of age or older whose
primary diagnosis is chemical
dependency;
2.
are willing and able to participate fully both, physically and mentally in the
program;
3.
are willing and physically able to secure and maintain gainful employment;
4.
have documentation which attests to appropriateness for the program which shall
include, but not be limited to:
a)
DSM IV diagnosis of chemical dependency as the primary disorder,
b)
Medical assessment to assure that there is no medical condition that would
impede participation in the program, or pose a health threat to other residents
or staff, and
c) a Social History to help establish
optimum treatment goals for the individual;
5.
have a minimum of 7 to 14 days freedom from abuse of chemically
addictive/dependent substances; and
6.
have sufficient sponsorship or financial resources to guarantee completion of
the program.
Staff of The Freeman Center work with each individual to help develop the most
beneficial sponsorship arrangements, whether it is full or partial sponsorship,
or arrangements for self-pay for the treatment services. The Freeman
Center is credentialed as a Medicaid Provider.
Determination
of the appropriate Level of Care (e.g., Detoxification, Intensive Residential,
Residential, Intensive Outpatient, or Outpatient Treatment) will be made in
accordance with Utilization Review Standards established by the Texas Department
of Insurance and/or Court Orders.
